common problem with steel production. Many checks are/should be done to avoid this, but it still happens.

I have messed up my hands many times, putting load on a tool only to have it snap and send my knuckles into an engine... I always inspect broken tools to try and figure out why/how they break, and almost always there is an air bubble in the center of the break. (so far all breaks of this type have been with Chinese tools)
