I've been making pancake style sheaths for many years now and and say with confidence that its one of the strongest ways to carry a tool, be it a folder, multi tool, or fixed blade. The style keeps its nice and tight to the body, and you can put it on the belt with a loop in between the ears so the sheath wont wander.
Some things to look for and to make your sheath as comfortable as possible: Take the tool in its present sheath and hold it to your belt, now you can angle it and play till you find a nice balance, this will be the angle. Make sure your maker is familiar with making the pancake sheath, theres a few variables in the construction and stress points that need to be addressed. The ears should be thick enough to hold up but not so thick that it hinders their ability to mold to the belt. The distance between the ears is something to ponder as well, to close and the opening wont be wide enough to allow the belt in, too wide and well, it just looks funky.
The one pictured looks good! Machine stitched though, <img src="/ubbthreads/images/graemlins/tongue.gif" alt="" /> But the design looks solid.
